Sertoma Golf Tournament

 

Thank you for your interest in the Sertoma Golf Tournament to be played at 1:00 p.m. on October 3, 2008 at Oak Valley Golf Club, sponsored by the Winston-Salem Sertoma Club.  Proceeds from this tournament will be used to provide funding for the charitable programs and activities of our club, including Camp Sertoma and the Sertoma Summer Speech Program.

 

The format will be a Captain’s Choice (Shot-gun start) and prizes will be awarded for the 1st, 2nd and 3rd place teams.  Other prizes will include a two-year car lease and a $2500 cash prize for holes-in-one on two of the par three holes. Lunch will be provided.  Please register no later than Friday, September 26, by printing and completing the application below and mailing it, along with a check for $65 per entrant (or $260 per team), to the following address:

Participation will be limited to the first 144 applicants, so early registration is advised.  Cancellations will be accepted up to one week prior to the tournament.  A portion of the cost may be tax deductible; please consult your tax advisor.
