Thank you for your willingness to support the 2011 Sertoma Golf
Tournament sponsored by the Winston-Salem Sertoma Club. Proceeds from
this tournament will be used to provide funding for the charitable programs and
activities of our club, including Camp
Sertoma and the Sertoma Summer
Speech Program.
The event will be held on May
6, 2011, at the Oak Valley Golf Club in Advance, North Carolina. Hole sponsorships
are $150 each and you can designate the hole(s) you wish to sponsor
(first-come-first-served). Please make
checks payable to the Winston-Salem Sertoma Club and mail
them, along with this completed form, to P.O. Box 15572, Winston-Salem, NC
27113. Your contribution may be
deductible for tax purposes, please consult your tax advisor.
