Thank you for your willingness to support the 2008 Sertoma Golf Tournament sponsored by the Winston-Salem Sertoma Club. 

Proceeds from this tournament will be used to provide funding for the charitable programs and activities of our club, including Camp Sertoma and the Sertoma Summer Speech Program.

This year’s event will be held on October 3, 2008 at the Oak Valley Golf Club in Advance, North Carolina.

Hole sponsorships are $150 each and you can designate the hole(s) you wish to sponsor (first-come-first-serve). Please make checks payable to the Winston-Salem Sertoma Club and mail them, along with this completed form, to P.O. Box 15572, Winston-Salem, NC 27113. Your contribution may be deductible for tax purposes, please consult your tax advisor.
